What is the Elevate Care Program?

Elevate Care Program is a clinician-supervised precision-health program designed to help care teams organize relevant patient context, personalize next steps, and support more connected care over time.

Why ECP exists

Many patients and care teams have access to more information than ever, but that information is not always easy to connect in the context of a real clinical workflow. Important details may live across history, laboratory information, lifestyle conversations, patient-reported experiences, and available monitoring sources. ECP is designed to help make that context more usable—so care teams can have more informed conversations, guide individualized next steps, and support continuity between visits.

What ECP does

ECP supports a care journey that begins with patient enrollment and relevant health context, moves through qualified clinician review, and continues through personalized next steps and follow-up. Technology can support organization and workflow, while clinicians remain responsible for patient care decisions.

What makes ECP different

ECP is built around clinician supervision. It is not designed to replace the patient–clinician relationship, automate medical decisions, or deliver care without qualified oversight. The clinician remains responsible for determining what is appropriate for the patient.

Who ECP is for

ECP is designed for clinician-led practices and organizations interested in a more personalized, longitudinal model of care, as well as patients participating through those care teams.
