A patient guide to participating in ECP.

ECP is designed to help your care team deliver a more connected, personalized care experience. Participation begins with a conversation with a participating clinician or practice.

What participation may include

Depending on your care team and your individual circumstances, participation may include conversations about your goals, review of relevant health context, clinician-guided next steps, education, follow-up, and ongoing engagement.

Your clinician remains central

Your care team determines what is appropriate for you. Technology may support the organization of information and care coordination, but it does not replace your clinician’s professional judgment.

Questions to ask your care team

• What is the purpose of ECP in my care? • What information may be considered as part of the program? • Who should I contact if I have questions? • How often will we revisit my care plan? • What should I do if I have an urgent health concern?

Safety note

ECP is not an emergency service. Contact your care team for individual medical guidance. If you believe you are experiencing a medical emergency, call local emergency services or seek emergency care immediately.
