For patients

Care built around your real life—not just your next appointment.

Your health is influenced by more than a single test result or a brief office visit. ECP helps your care team consider relevant context, guide personalized next steps, and stay connected as your needs, goals, and circumstances evolve. The program is clinician supervised. Your care team remains responsible for your care and helps determine what is appropriate for you.

What a more connected care experience can feel like.

See more of the picture

Your care team may consider relevant health history, available clinical information, your goals, your daily context, and other information that can support a more complete conversation.

Understand your next steps

Your clinician can help translate what matters into clearer, more personal actions and follow-up priorities.

Stay connected over time

ECP is designed to support meaningful engagement between visits, so your care plan can be revisited as your needs and goals change.

Keep your clinician at the center

Technology may support organization and communication, but qualified clinicians remain responsible for decisions about your care.

What to expect

Your ECP journey begins with your care team.

1. Talk with your care team

Your clinician or care coordinator explains the program, answers questions, and helps you decide whether participation may be appropriate for you.

2. Share relevant context

Your care team may review information that is relevant to your care, such as your health history, available test results, personal goals, lifestyle context, and other approved information sources.

3. Receive clinician-guided next steps

Your clinician reviews your situation and may recommend appropriate actions, education, follow-up, monitoring, or coordination with other members of your care team.

4. Revisit progress over time

Your needs and goals can change. ECP supports ongoing communication and follow-up so your care team can revisit the plan when appropriate.

What ECP can—and cannot—do.

ECP can support • More personalized discussions with your care team. • A clearer understanding of next steps. • Ongoing engagement and follow-up. • Clinician-guided care planning that can evolve over time. ECP cannot replace • Your clinician’s judgment. • Urgent or emergency medical care. • The need to contact your care team when you have a medical concern. • Medical advice tailored to you outside of your relationship with a qualified clinician.

Your information deserves care, too.

Your participating care team can explain how information is used in the context of your care and answer questions about participation. Please do not submit medical details or personal health information through general website forms.

Participation

ECP participation begins with a participating clinician or practice.

If your care team has introduced ECP, they can explain next steps and answer questions about your individual situation. If you are interested in learning whether ECP may be available through your practice, contact your clinician or care coordinator.
