For healthcare practices

Turn more of your care team’s work into a connected, proactive patient experience.

ECP helps practices structure a more personalized, clinician-supervised model of longitudinal care. It is designed to make relevant patient context easier to review, care pathways easier to coordinate, and ongoing patient engagement more practical to deliver. Your care team remains in control. ECP supports clinical workflow; it does not replace clinical judgment.

Designed for practices building a more continuous model of care.

ECP may be relevant for clinician-led practices that want to strengthen patient engagement, coordinate personalized care over time, and create a more structured approach to proactive follow-up. Program fit is determined through a practice-readiness conversation. ECP may not be appropriate for every practice, patient population, workflow, or market.

Primary and longitudinal care

Practices seeking a more connected approach to patient engagement, preventive care, and ongoing clinical follow-up.

Cardiometabolic and lifestyle-focused care

Teams supporting patients whose care benefits from ongoing attention to health patterns, behavior, education, and coordination.

Integrative and personalized-care practices

Clinician-led practices that want to connect individualized care planning with a more repeatable patient journey.

Innovation-ready organizations

Clinical groups and health systems evaluating new models for clinician-supervised, data-informed longitudinal care.

What ECP can support

A more practical operating model for personalized care.

Clearer clinical context

Bring relevant health history, available information, patient goals, and ongoing context into a more useful clinical conversation.

Repeatable patient journey

Establish a structured pathway from enrollment and clinician review to personalized next steps and follow-up.

Longitudinal engagement

Create a more consistent rhythm for patient education, care-team touchpoints, monitoring where appropriate, and plan refinement.

Clinician-directed support

Use technology-supported insights to inform care without giving up clinical authority or patient-specific judgment.

Care-team coordination

Give clinicians and staff a more consistent framework for organizing activities around the patient’s evolving needs.

Practice-readiness visibility

Evaluate patient-panel opportunity, workflow fit, staffing requirements, and implementation priorities before launch.

From interest to launch

A structured approach to implementation.

ECP is designed to be introduced through a deliberate, clinician-led process. The exact scope, timeline, and implementation plan are tailored to the practice and its readiness.

1. Assess fit

Start with a conversation about your patient population, clinical goals, operating model, and readiness for a longitudinal program.

2. Define the workflow

Work with the ECP team to identify how patient enrollment, clinical review, care-team coordination, and follow-up may fit into your practice.

3. Prepare the team

Align clinical and operational stakeholders around program roles, patient communication, and implementation priorities.

4. Launch intentionally

Begin with a defined patient population and a practical operating rhythm designed for learning and refinement.

5. Review and evolve

Use implementation experience to improve adoption, patient engagement, care coordination, and program fit over time.

Designed to support—not displace—clinical judgment.

ECP keeps the practitioner and care team at the center of decision making. Technology-supported insights can help organize relevant context and surface care opportunities, while qualified clinicians determine what is appropriate for each patient. This clinician-supervised model helps preserve accountability in the chart, clarity in the care relationship, and control where it belongs: with the treating care team.

Practice opportunity

Evaluate the operational opportunity behind longitudinal care.

ECP can help practices assess whether clinically necessary, documented, and separately reportable work within a longitudinal care model may align with existing reimbursement pathways. The purpose is not to create a new billing category, but to help practices evaluate the work they are equipped to deliver responsibly.

The ECP Practice Cohort is for teams ready to evaluate program fit, plan a clinician-supervised workflow, and explore a defined initial patient population.

Practice FAQ

What types of practices may be a fit for ECP?

ECP is intended for clinician-led practices and organizations seeking a more personalized, longitudinal care model. Fit depends on the practice’s clinical goals, patient population, staffing, workflows, market, and implementation readiness.

Does ECP replace our existing clinical workflow or clinical judgment?

No. ECP is designed to support the care team with a structured, clinician-supervised workflow. Qualified clinicians remain responsible for clinical judgment, care decisions, patient communication, and documentation.

Does ECP guarantee reimbursement or revenue?

No. ECP does not create a new billing code and does not guarantee coverage, payment, collections, reimbursement, profitability, or financial results. Any reimbursement depends on the services actually provided, medical necessity, documentation, eligibility, payer rules, and other applicable requirements.

How does a practice begin?

Start by requesting a live demo or applying for the ECP Practice Cohort. The ECP team will use an initial conversation to understand your goals, readiness, and potential fit.
